New Form Builder Features & Release of Information (ROI)

You can now create and send Release of Information forms in SimplePractice!

Our form builder has always been a great way to create your own intake forms (or progress notes) and send them to your clients before their first session—now there’s even more you can do to customize all of your notes and forms.

We spoke with and read comments from hundreds of customers to determine which updates you valued the most. While a signable ROI was the top request, customers also requested additional tools to provide them with even more flexibility when creating their notes and forms. We’ve added these enhancements and renamed the existing question options to make the tools as clear and easy to use as possible.

We also added a new ROI template that you can use as is, or edit and make your own.

Here are some of the new highlights:

Text & Dividers

Make your forms easier for clients to follow. Add text or instructions anywhere on your form (for example, introductory text at the top). Create section headers and add line-breaks to segment sections for better readability and flow.


Clients can now fill out information on a form, then check a box to provide their consent (eSignature). This feature finally makes Release of Information forms available to use in SimplePractice.

Required Fields

You can now make specific questions required in order for your clients to submit their forms. If there’s a part of your intake that clients have skipped in the past, this will encourage them to read more closely and prevent them from submitting incomplete forms.

Question Type Updates:

We’ve added some new question types and revised and re-named the existing question options to make it clearer what to select as you build your form. The table below provides a complete list of all the updates, with descriptions:

Question Types Description
Long Answer Use this question type for more open-ended questions that may result in responses of a few sentences or paragraphs.
Short Answer This question type gives clients a single-line text input area for shorter responses.
Single Choice (Radio Buttons) This type is used for questions that allow only one response.
Multiple Choice (Checkboxes) This question type allows clients to select more than one response.
Dropdown Dropdown questions are used for single-select questions, similar to the Single Choice (Radio Buttons) question type. The primary difference is that the Dropdown question type takes up much less space.
Date Field This question type gives clients a date picker calendar to select a date.
eSignature The eSignature question type is only available for Intake forms. This question type allows you to require a client’s electronic signature.
