Alcohol use disorder ICD-10 diagnostic criteria

Alcohol use disorder is defined in the DSM-5 as a problematic pattern of drinking leading to significant impairment or distress in the last 12 months.

In addition, a person must have at least two of the following symptoms:

  • Drinking in larger amounts or over a longer period than intended

  • Desire to stop or unsuccessful efforts to quit

  • Significant time spent obtaining, using, and recovering from drinking

  • Cravings

  • Recurrent use causing disruption and inability to work or fulfill other major obligations

  • Continued use despite the impact to social and relational problems caused by alcohol

  • Important social and recreational activities are reduced or given up in favor of alcohol

  • Recurrent use despite being in dangerous situations or environments

  • Use is continued despite knowledge of having a physical or psychological problem either caused or exacerbated by alcohol

  • Tolerance to alcohol by needing increased amounts to have the same effect or a diminished effect despite drinking the same amount

  • Withdrawal symptoms when stopping use

According to the DSM-5, you can also specify severity based on the number of symptoms:

  • Mild (2-3 symptoms): F10.10 (uncomplicated), F10.11 (in remission)

  • Moderate (4-5 symptoms): F10.20 (uncomplicated), F10.21 (in remission)

  • Severe (6+ symptoms): F10.20 (uncomplicated), F10.21 (in remission)

While moderate and severe alcohol use disorder share the same F10.2 codes, clinicians should document the specific severity level in their clinical notes, as the ICD-10-CM system does not provide separate codes to distinguish between these two severity levels.

In your notes, you may also specify if the person lives in a controlled environment where access to alcohol is limited, such as a rehab or sober living facility.


Differential diagnoses 

When evaluating a patient for alcohol use disorder, clinicians should also consider other conditions that may better explain the symptoms or co-occur with problematic alcohol use. 

These include:

  • Other substance use disorders: Such as sedative, hypnotic, or anxiolytic use disorder, which can present with similar symptoms

  • Alcohol-induced disorders without use disorder: Including isolated episodes of intoxication (F10.920) or withdrawal (F10.930) that don't meet full criteria for alcohol use disorder

  • Co-occurring psychiatric disorders: Such as mood disorders, anxiety disorders, antisocial personality disorder, or conduct disorder, which may involve alcohol as self-medication

  • Medical conditions: Including liver disease, pancreatitis, or neurological disorders that may be caused by or contribute to alcohol use
